The latest free update for Fallout 76 lets players play as a Ghoul for the first time in the series’ history. Becoming a Ghoul is one of the big features of the Fallout 76 Ghoul Within update.
To become a Ghoul, players need to reach Level 50 first to unlock a new quest to turn yourself into a ghoul—irradiated humans cursed with looking like an undead but ironically blessed a long life. Unless it’s the feral variety, where they are turned into fodder enemies.
If you play as a Ghoul, NPCs and factions will react differently than your previous Wastelander self. So you can put on a disguise to interact with factions.
Ghouls are naturally immune to radiation damage (they’ve already suffered enough radiation to turn into ghouls). Instead, radiation heals them.
But instead of a hunger and thirst meter, Ghouls need to balance the Feral meter, where there is a risk/reward for leaving yourself intentional going Feral over time.
Ghouls get 30 new Ghoul-specific Perk Cards.
Ghouls can revert into their normal selves via an in-app purchase from the Atomic Shop.
The Ghoul Within update also brings Season 20: Glow Of The Ghoul. The new season adds more ghoul-themed rewards and items for C.A.M.P. for players to unlock.

Fallout 76 may have had a terrible launch and even worse reception, but the live service multiplayer Fallout game manages to survive this long after all those terrible review bombs were dropped. Rather appropriate that this is the entry to the series where you can play as Ghouls if you ask us. The game has come a long way.
